Mesh entities are now sorted locally following the same ordering
principles as in FIAT. Only implemented for triangles at this point.

This means that all vertices are ordered counter-clockwise on each
triangle and vertex i is always opposite to edge i in a triangle.

Seems to work well with mesh refinement, but I have not done any
extensive testing.
