[Bug 1451284] Re: USB dies ("frame counter not updating") running Ubuntu 15.04 in Virtualbox 4.3.26

Florian, thank you for reporting this and helping make Ubuntu better.
Could you please test the latest upstream kernel available from the very
top line at the top of the page (the release names are irrelevant for
testing, and please do not test the daily folder) following
https://wiki.ubuntu.com/KernelMainlineBuilds ? It will allow additional
upstream developers to examine the issue.

If the test did not allow you to test to the issue (ex. you couldn't boot into the OS) please make a comment in your report about this, and continue to test the next most recent kernel version until you can test to the issue. Once you've tested the upstream kernel, please comment on which kernel version specifically you tested. If this bug is fixed in the mainline kernel, please add the following tags by clicking on the yellow circle with a black pencil icon, next to the word Tags, located at the bottom of the report description:
kernel-fixed-upstream
kernel-fixed-upstream-X.Y-rcZ

Where XY and Z are numbers corresponding to the kernel version.

If the mainline kernel does not fix this bug, please add the following tags:
kernel-bug-exists-upstream
kernel-bug-exists-upstream-X.Y-rcZ

Once testing of the upstream kernel is complete, please mark this bug's
Status as Confirmed. Please let us know your results.

** Tags added: regression-release

** Changed in: linux (Ubuntu)

Title:
USB dies ("frame counter not updating") running Ubuntu 15.04 in
  Virtualbox 4.3.26

Status in linux package in Ubuntu:
  Incomplete

Bug description:
Running Vivid Vervet in Virtualbox 4.3.26 (on Windows 8.1) with latest
extensions installed, clean Ubuntu install, accessing an external hard
drive via USB, mounting an encrypted Truecrypt volume (and ntfs-3g).
All seems fine (mounting no problem; opening various files) until
putting some load onto the system (copying 2GB from the Virtualbox
shared folder to the USB drive), at which point the following happens
  (from syslog):

  May 3 15:57:41 florian-VirtualBox kernel: [ 707.710825] ohci-pci 0000:00:06.0: frame counter not updating; disabled
  May 3 15:57:42 florian-VirtualBox kernel: [ 707.711080] ohci-pci 0000:00:06.0: HC died; cleaning up
  May 3 15:57:42 florian-VirtualBox kernel: [ 707.712771] usb 1-1: USB disconnect, device number 2
  May 3 15:57:42 florian-VirtualBox kernel: [ 707.722250] sd 3:0:0:0: [sdb] FAILED Result: hostbyte=DID_NO_CONNECT driverbyte=DRIVER_OK
  May 3 15:57:42 florian-VirtualBox kernel: [ 707.722264] sd 3:0:0:0: [sdb] CDB:
  May 3 15:57:42 florian-VirtualBox kernel: [ 707.722380] Write(10): 2a 00 61 b8 07 70 00 00 f0 00
  May 3 15:57:42 florian-VirtualBox kernel: [ 707.722614] blk_update_request: I/O error, dev sdb, sector 1639450480
May 3 15:57:42 florian-VirtualBox kernel: [ 707.722891] Buffer I/O error on dev dm-4, logical block 203620366, lost async page write

... followed of course by loads of I/O errors.

Didn't experiment much with trying to get the drive back online, but
shutting down Virtualbox, unplugging and re-connecting the drive, then
starting VB and Ubuntu again, does the trick every time. However, the
  same problems keeps recurring.

This problem does not exist in an otherwise identical setup with
Ubuntu 14.04. (Same version of Virtualbox to host Ubuntu; same version
of Truecrypt; but I don't think TC can be to blame anyway, since this
seems to be happening at the device level)
