Christopher,

1) Yes, badblocks scan shows no errors
2) I make bootable USB stick from ISO using dd. I use another USB stick for that (not Sandisk). After booting I proceed with installation process - I use Sandisk USB as target as if it was a regular harddrive. Installation proceeds without problem. Than I boot from Sandisk and this is when the problem occurs.
3) It was few month ago but I think it was ubuntu-15.10-desktop-amd64.iso.

Than I upgraded to 16.04 using update-manager.

  Ubuntu 15.10 fails to boot from Sandisk USB stick

  Incomplete

Bug description:
  Booting from Sandisk USB stick fails and stops on "device descriptor
  read/64, error -71" and "device not accepting address 6, error -71" on
  Dell Latitude E5420 Laptop. I tried every USB socket but without
  success. Booting with "rootwait" parameter does not help either.
