[Bug 1342021] [NEW] rt2500pci: phy0: rt2x00queue_flush_queue: Warning - Queue 0 failed to flush

Public bug reported:

In Trusty i386, Ralink RT2500 wireless card looses connectivity one or two minutes after every boot.
A warm reboot of the system is enough to replace the card in working mode, again for just a minute or two.

The same system has been used for a long time with Precise without
problems, and also had other various non-LTS releases tested without
problems. I'm sorry I cannot tell for sure which ones were tried in
between Precise and Trusty.

As far as I know, this is the same issue as Ubuntu bug #1318837 and
Linux bug https://bugzilla.kernel.org/show_bug.cgi?id=61621

I tested some kernels on my system with these results:
- 3.13.0-30-generic (Trusty's current stock kernel) is affected
- 3.13.11-03131104 is affected (and so are a few other 3.13.x that I tested)
- 3.14.1-031401 works (and so do all 3.14.x at least up to 3.14.9-031409)
- 3.15.4-031504 works
- 3.16.0-031600rc2 and rc4 both work
