I've tried this on dogfood.  It takes A2 from around a minute to five seconds without the index from https://code.launchpad.net/~cjwatson/launchpad/db-optimise-publish-a2/+merge/226798 or less than a second with it.  Total dogfood publisher runtime for a no-op run is now under a minute, most of which is germinate, although maintenance-check is currently broken on dogfood so that time isn't entirely representative.
