    Use keys in admin menu arrays (bug #799989)
    
    This makes it easier to delete & change the order of menu items inside
    the local_main_nav_update function.

Bug description:
  The admin & institutional admin menus can't be added to by plugins,
  and are not so easy to edit with the local_main_nav_update function
  because the menu arrays don't have keys.
