I've actually moved on to a new role, but the issue would still be of value to my previous institution.
The groups were created manually and until I attempted to add users in this way they had all been added via the mahara members interface within each group - which is time consuming (and frustrating if you are sitting with a list of names).
I only discovered the problem because it threw an error message about institutions when I tried to upload a csv as site admin. I can't recall exactly what is said - possibly Institution was null, so when I looked in the database I discovered that the institution field within each group was blank - when I added a value the csv worked.

  Institution data not added when creating groups

Bug description:
  While trying to add group members by CSV I discovered today that none
  of our groups on Mahara were assigned an institution on our mySQL
  database - either those migrated from previous Mahara 1.4 or any added
  since in 1.5 or our current 1.6.2 version.

  I can manually add the record to each group as the institutions are
  listed in a dropdown field and should be able to script replace any
  blank values with a default one (if anyone can suggest the script that
  would be great, otherwise I'll figure it out eventually), but moving
  forward is this a bug specific to our installation or more widespread
  - and what would we need to do to fix it so that future groups are
  automatically assigned into the correct institution?
