Cheers for the bug report. I imagine this would be relatively easy to
implement... but slightly bigger than a "bite-sized" bug. I'll tag it
with the new "snack-sized" status.

  Upload group users from csv should respect users 'uniquebyusername'

Bug description:
  The current version of the upload requires that the institution name
  is available for each username. If $cfg->usersuniquebyusername is
  enabled there is no requirement for this (as usernames are guaranteed
  unique across the site). Moreover, it probably isn't known what
  institution they are currently attached to making the CSV upload
  unusable.
