Public bug reported:

The current version of the upload requires that the institution name is
available for each username. If $cfg->usersuniquebyusername is enabled
there is no requirement for this (as usernames are guaranteed unique
across the site). Moreover, it probably isn't known what institution
they are currently attached to making the CSV upload unusable.
