No changes have been made to the default theme since Mahara 1.5 was
first installed. I did a fresh install and the default theme appears to
work and all buttons present. We use MySQL 5.5 for the database which is
the only difference. I guess something strange has happened during an
upgrade.

Did you make any changes to the default theme, Richard? There is no
problem with the default theme on http://demo.mahara.org which runs on
Ubuntu server and Postgres though. I don't know if a different database
would be an issue, but it would be good to rule out that you do not have
theme or otherwise any customizations on your site. If you do, can you
please make a fresh install of Mahara on the same server and see if the
problem occurs as well?

  OS: Debian Squeeze Linux 32bit
  Database:  MySQL 5.5.20 for Windows
  Browser:  Chrome 32, Firefox 23 and Internet Explorer 10

  When attempting to edit a page, the buttons for Media, Journals,
  General, Personal Info and External don't appear but instead there is
  a text link called 'Blocklist type' which expands to show buttons as
  text links, when any of these links are clicked, it resets the menu
  view back to default.

  Upon changing the theme to anything else i.e. Raw, or Aqua then the
  buttons for all blocktypes categories appear and can be expanded to
  allow drag and drop of specific blocks i.e. Media --->  Files to
  download

  The following error message is only displayed when using no theme or
  default theme:

  A required parameter is missing or malformed Missing parameter 'c' and
  no default supplied

  I have attached a screenshot to demonstrate the issue.
