Rearranged table headers in Edit access (Bug #1262487)

Collapsed the two header rows into one, added hidden headers where
appropriate and added labels to the "Comments" checkboxes.

  Row headers in "Edit Access" table are confusing for screen reader
  users

Bug description:
  When sharing a page or collection (/view/access.php), the row headers in the column are overly complex and confusing for people navigating using a screen reader.
  Currently there are two rows of <th> cells with different information in each row and various blank cells. These should be combined into one single row with headings like "Accessible from:", "Accessible to:", and hidden header cells where appropriate (eg. "Editing buttons" for the last row)
