Use labels in 'files' pieforms element (Bug #1277290)

Added hidden labels to each file input and changed the [+] button to use
the standard add icon with accessible alt text.

  Resume attachments form element should be made accessible

Status in Mahara ePortfolio:

Bug description:
  The 'files' form element used to attach files when adding (but not
  editing) Resume education history, employment history, etc. is not
  currently accessible. Ideally this element should be removed entirely,
  but if that cannot easily be done then the following changes need to
  be made:

  - The [+] button needs to be an image with descriptive alt text
  - Each file input needs a hidden label

  Affects latest master
