It's a very old wish but I think you may now use
https://wiki.mahara.org/index.php/Plugins/Artefact/Booklet
for such a task.
We use it since one year and I have implemented Mahara 10.1 version.

Once Booklet structure is designed, anybody may use it to type in data
and publish them in his portfolio.
Not so different from what you look for, except that you have to give right to create form structure to identified users.
Institutions support seams to be easy.

Interface to create custom form artefacts

Bug description:
For Mahara to be useful we would need to ability to create completely
custom forms for students to enter data. I imagine that these things
would be artifacts of some sort. The main features of this would be as
follows...
* Artifact is a simple collection of form fields that are available to students to fill in.
* The can be included in a view so that students can share and reflect
* They need to be created by non technical users, so some simple builder mode would be needed. This needn't be too complicated. Probably just a list of fields with names and descriptions with "add new"/"delete"/"edit" options.
* They need to be attached to institutions somehow. That is only students from a particular institution can see "their" custom forms.
We might be able to put some effort into this, but I guess the big questions are:
* Is there a mechanism to control artifacts and views on an institution by institution>
* Does the artifact model easily (or at all) permit simple artifacts to be created "virtually" - some sort of database construct or (worse) serialised object as opposed to a block of code?
