[Bug 1690979] [NEW] Create a page when you create a journal

Making a journal available entails three steps:
1. Creating the journal and writing the journal entries.
2. Putting the journal into a page.
3. Make the page accessible.
People are often used to blogging software where they write their
entries and they are instantly published. For Mahara, we do have to
consider the sharing step. However, we could simplify the workflow to a
degree by creating a page as soon as you have a journal available so
that you then only have to decide on the access. That way, 1 step would
be removed without jeopardizing privacy.
The expected behavior would be the following.
Site admin "template":
1. The page "Journal" is a template like "Dashboard" and "Profile" so the site admin can change the default settings, e.g. the layout.
2. The default layout for the page is 1 column across the entire site.
New user account:
1. User account is created and with it the default journal. (existing behavior)
2. Automatically, the page "Journal" is created alongside "Dashboard" and "Profile". But the page is not shared with anyone. The page has the layout as defined in Administration -> Configure site -> Pages and collections.
User account with multiple journals turned on:
1. User turns on multiple journals in the account settings. (existing behavior)
2. User creates a new journal. (existing behavior)
3. The page "Title of the journal" is created automatically based on the "Journal" template. It is not shared with anyone.
