Title:
  Grades via LTI 1.0 are not getting into the Moodle gradebook

Bug description:
  When the external LTI tool is set up to push grades for a portfolio to
  Moodle, the grade never arrives in Moodle. It is only displayed in
  Mahara.

  To test in Mahara 20.10 (21.04 doesn't display the grade option, but I
  suspect it has the same issue; separate bug #1941663)

  1. Set up the portfolio submission to Moodle as described in the manual: https://manual.mahara.org/en/20.10/external/externalapps.html
  2. Submit a portfolio as a student coming from Moodle.
  3. Grade the portfolio in Mahara as a teacher. The grade is displayed.

  Expected result: The grade is also displayed in the Moodle gradebook.
  Actual result: It is not displayed, not even waiting a few minutes in case a cron needed to run first.

  When you go to the web services logs in Mahara, you see the attached:

  Even though the web services user is in an institution, it says 'root'
  and there is no mention of an institution. The error message is
  "exception: WebserviceAccessException message: Access control
  exception : Invalid token selected or none supplied debuginfo: "

  Interestingly, the grade is pushed to Canvas. It just seems that
  Moodle is having a problem.
