Actually this is very much what we are doing.

While this reference is decidedly not up to date.  It does point out that
XML is a widely used technology.  By building the CCDs on XML schemas.  We
must remember that these simply define the IDEAS of medical concepts.  The
actual data are XML documents.  This is the important concept to understand.
