But, according to this paper Tim sent me, it is better than free text notes:

The paper above suggestes that at least dictating notes is the worst choice!

OK that their outcome was not "lost semantics", but "quality of care",
but we are assuming (maybe a very strong assumption) that keeping
semantics is an intermediate step to improve quality of care.

Semantics is lost everywhere, which makes this project huge...

Do you mean that there is a relationship between the database type or
software functionality to the components or assertions of the CCD?
