[Bug 1037155] [NEW] Datatype for DvOrdinal.dv is incorrect

Public bug reported:

All releases 2.3.0 and prior.

This was originally established as an integer. However, as noted in
this openEHR bug report http://www.openehr.org/issues/browse/SPECPR-44
as well as an issue concerning the mini-mental state exam brought by
Flavio Seixas; this datatype should be 'decimal' from the assumed types
(a Real in many implementations).
