Public bug reported:

I created a subscruption, after the scheduled action run, no document
created. Below is the steps.

1) Create a new Document Type with "Accounting Voucher" object , Active and no Document fields
2) Create a new Journal Voucher with Cash Journel, state = post, amount = 10.00
3) Create a new Subscription with 

Interval Qty : 1
Interval Unit : Months
Number of documents : -1
First Date : 	02/03/2012 22:46:37
Source Document :Step1's Document and Step 2 's Cash Journel item and Amt = 10.00

then click process. A scheduled action created , i ticked Repeat Missed and changed the Next Execution Date to 21/03/2012 18:27:37.
after 21/03/2012 18:27:37 the Next Execution Date changed to 21/04/2012 18:27:37 (meaning that task run).
However, i go to Journel Voucher screen where no such item created even the Journal Entries. Check from the Sunscription's "Documents Created" where show nothing too.
