Windows keeps a log, and I'm hoping that this may shed some more light
on the issue.

First off, start OpenLP and wait for it to fail.
Now follow these instructions to open the Event Viewer: http://windows.microsoft.com/en-gb/windows7/open-event-viewer
Next you need to look for an entry who's time matches the time that OpenLP failed. (probably under Application) It might not necessary have the name OpenLP associated with it.
Finally double click this entry and copy the information in to a reply.

Note: My instructions are based upon Windows XP, so may differ slightly.
