  Presentations/Images with same name gets the same thumbnail

Status in OpenLP - Worship Presentation Software:
  Confirmed

Bug description:
  Noticed the issue on Linux but should be reproduce-able on all
  platforms.

  Found in trunk.

  Steps to reproduce:
  * Create 2 different images/presentation but with the same name, and (of course) in different folders.
  * Load both into the mediamanager.
  * For images they get the same thumbnail. For presentations the second will fail to load with error: "Duplicate files found on import and were ignored"
