Kage,
Any more immediate solution like this one is one that would need to be 
implemented in Wordpress. However, if you would like to have admin-type 
access to the Drupal installation get with me. The current focus with 
Drupal is 2-fold: configure the theme (which is pending a proper draft 
from our designer) and configure CiviCRM. Any 
suggestions/additions/improvements to the setup are welcome as well.

I have created the form and made you the owner. You can see the 
Wordpress draft at http://theosdf.org/?page_id=133&preview=true (you 
must be logged in). Let me know if you need help with anything like 
adding it to a menu, etc.
