Keyboard layout breaks randomly - all keys on top left corner of the
  keyboard

Status in “ubuntu-keyboard” package in Ubuntu:

Bug description:

There is no specific test to reproduce this behavior and seems rather
random when the focus enters an input field and first time the
keyboard is displayed. I got this issue 3 times this morning with
  20130918.on mako

All the keys are on the top left corner cf screenshot attached
