[Bug 1078498] [NEW] /debian/patches are used only to modify upstream files

Public bug reported:

The guide should tell that /debian/patches and $ edit-patch should only
be used when trying to modify upstream source, for changes in the
/debian/ dir they're usually made directly, otherwise, you may find
errors like this one:

http://paste.ubuntu.com/1356603/ - error
http://paste.ubuntu.com/1356606/ - patch who modify /debian/rules

** Affects: ubuntu-packaging-guide
     Importance: Undecided
         Status: New
