[Bug 425992] [NEW] Screen fades white to black, xorg crashes on boot

Public bug reported:

Binary package hint: xserver-xorg-video-intel

Boots up, the ubuntu loading screen bar fills up, and when it should go
to the login screen, the screen turns white and fades to black in
patches. Sometimes it flashes and starts again, and you can briefly see
a cursor or spinning thing. Often it won't let you open a terminal, and
other times it will open a terminal, but the screen isn't set up and it
flashes quickly and isn't lined up very well. If you don't open a
terminal, you can type your login credentials blindly and hear it
continue booting. I've tried 8.04, 9.04 and 9.10, all with the same or
similar results. Attached is the output of ubuntu-bug -p xserver-xorg-
video-intel, I will follow up with Xorg.0.log, xorg.conf, dmesg,
messages and lspci -vv. Here is apt-cache policy xserver-xorg-video-
intel:

xserver-xorg-video-intel:
  Installed: 2:2.6.3-0ubuntu9
  Candidate: 2:2.6.3-0ubuntu9.3

        500 http://us.archive.ubuntu.com jaunty/main Packages

And lsb_release -rd:

Description:	Ubuntu 9.04
Release:	9.04

Thanks!
