I feel everyone should enjoy the hilarity of this:

Ignore my last statement. I was about to change graphics cards to see if
the problem could be replicated on another card, when the VGA connection
fell out of the back of my monitor. I suspect that the connection
between the cable and monitor was marginal, and that this was the cause
of the problem.
