I seem to have an issue quite similar to the following question:

There are some slight differences. First, I running Hardy (8.04). Second, this issue appears to occur more than 50-80% of the time; it always happen on cold boots, and 75-80% on hard resets. Third, I am unable to login via ssh after this issue occurs.

Description:
Grub comes up, boots default, usplash progress bar moves across screen, screen goes "light" black, ubuntu busy cursor displayed, ubuntu regular cursor displayed, screen goes "deep" black, monitor OSD comes up saying resolution doesn't match native 1280x1024.

Installed Ubuntu on the 1st of June. First started noticing this issue the week of June 15th to 21st.
