Ok, there are several different types of ways of doing this in Writer

1. Press F1 to get help, click on the "Index" tab and search for "tick
box creation"

2. Click on "Show Draw functions", it looks like a paint-brush on the
top icon bar.  Now at the bottom of the window click on the blue square,
the mouse arrow changes to a cross - use it to set opposite corners of a
large square.  It fills with blue by default.  There should be a new
toolbar at the top with 2 drop down lists labelled "colour", roll the
blue one up quite a few places, just above the greys is white.  Then
resize your box.  Use copy and paste to create as many as you need :)
Dragging them into place is tough, good luck with lining them all up :(
This is why option1 suddenly looks more attractive ;)

3. If you needed larger boxes for people to write in then perhaps
consider using table and borders

4. I tried using bullets&numbering but couldn't find an empty box.

If you can get option4 to accept a box you've drawn elsewhere as a
graphic then this would be the best option, probably.  Otherwise i would
go with option2 this time and just accept it's not going to be perfectly
aligned - and then i would play around with option 1 in odd spare
moments so i could be ready for next time
