OK, thanks for the clarification Francis.

As a team, we'll make the occasional mistake. What matters is that we
learn and improve, and that we never let an issue fester without
figuring out who is responsible for the fix. Sometimes that will be
tooling, sometimes it will be a social agreement on process.
