Title:
  (service_type, provider_name) pairs should be unique

Status in OpenStack Neutron (virtual network service):
  Invalid

Bug description:
  The recently reviewed service type framework allows for specifying in
  the configuration file multiple times the same service provider for
  the same service type.

  This means that different drivers might end up being stored for the
  same (type, provider) pair, which might cause issues with driver
  selection.

  for instance:

  lb:my_provider:driverxxx
  lb:my_provider:driveryyy

  should be disallowed.

  If this is agreed, it might be easier to just disallow duplicate
  (service_type, provider_name) pairs
