[Bug 1401954] [NEW] Floating IP association dialog displays "None" for loadbalancer pool vips

Public bug reported:

For instances, the "Manage Floating IP Associations" dialog display the
instance name, but for LB pool VIPs, it displays "None".

Steps to reproduce :
- Create a LB pool.
- Add a VIP to that pool
- Allocate a floating IP
- Click associate for that floating IP to open the dialog

Result: In the "port to be associated" field, the vip is displayed as "None: 10.0.0.5". See the attached screenshot.
Expected result: the name of the vip, or the name of the pool should be displayed.

I have a patch ready, but I'd feedback on a point: should it display to pool name, or the vip name ?
