    Clean up Image Registry form tag fields
    
    This patch updates the tag fields on the Image Registry form so they
    have labels and use the form-group class. It also updates the "Add"
    buttons so just an icon is displayed. This will fix problems with
    translated text making the button too wide.

  "Image Registry" window contains a text box which overlaps with
  buttons (Japanese and German)

Bug description:
  Project > Data Processing > Image Registry > Register Image

  The "Register Image" window, "Image Registry tool" text box becomes
  larger when it contains translated text (only confirmed in German and
  Japanese, but could be affecting more languages). It overlaps with
  buttons and other text.
