I also don't see cloud-init running in the guest console dump, so it
appears that this server just never booted properly.

  It happened on master (Queens).

  The last messages in the instance console log are:

  [    3.395758] Freeing unused kernel memory: 928k freed
  [    3.418579] Write protecting the kernel read-only data: 12288k
  [    3.525176] Freeing unused kernel memory: 1596k freed
  [    3.589609] Freeing unused kernel memory: 1184k freed

  info: initramfs: up at 3.71

  So it seems that no getty was started on tty*.

  Nothing clearly suspicious in nova-cpu service log or syslog.
