I think that this may be coming from duplicate names for some of entity
provided (image, flavor, key, etc). It is not reproducible in
environment without them. Closing this as bug was in incomplete state
for a year.

  Steps to reproduce
  ==================
  I use demo scipt to launch an instance:

  $ . demo-openrc
  $ openstack server --debug create --flavor m1.nano --image cirros --nic net-id=698c77d5-49cb-47f2-8e26-766b2be3783d --security-group default --key-name mykey selfservice-instance

  Expected result
  ===============
  An instance created and show status like https://docs.openstack.org/install-guide/launch-instance-selfservice.html

  Actual result
  =============
  An instance created but pop up Python error:
