Install read-edid, then run get-edid while the monitor is connected.
Does it report an error or is the external monitor detected correctly?

If it is not detected correctly, check the output of
  lshal|egrep '^  system\.(kernel|firmware)\.'

If system.firmware.version is '212' then have a look at
