Thanks everyone.
What require to be change in the system is Based on number of registered
data values :-

   1. Actual= Total facilities/communities reported/entered into the system
   2. Target = Expected health facilities/communities to report
   3. 1st percentage = % complete reporting
   4. on-time = Total facilities/communities entered on time
   5. 2nd percentage = % timely entered reports
