[Bug 950227] [NEW] Use 'bzr lp-propose' instead of 'bzr lp-open'

Some feedback that I received from a new contributor reminded me that
using 'bzr lp-propose' instead of  'bzr lp-open' where appropriate in
the documentation simplifies some tasks. We should review this usage.
