The thing is, the spreadsheet is known to be largely unreliable, but
the problems you've described do not involve the spreadsheet, so that
terrifies me because everything outside of the spreadsheet should be
simple and reliable. I'm not aware of *any* circumstances that could
have caused the issues you describe and I've never seen any similar
behaviour myself.
