Can you please attach "lsusb -v" recorded while the mouse is connected to the machine?
Also, please save the output of "dmesg" after connecting the mouse and attach that as well.

Is this mouse an USB mouse that you occasionally use together with a USB
-> PS/2 converter?

Can you provide a link to exact logitech wireless mouse you're using? (a
link to a webstore or something that carries some basic technical data
would be useful).

Which are the xserver, kernel and evdev versions you're running in
Gentoo? (the bits that work)

Finally, can you please run this debug analysis tool? Basically you need to run
"find /dev/input/event*" before and after plugging in the device and that way you can figure
out what its "/dev/input/eventX" entry is and then you pass that to the evtest program.
It would be useful to have evtest dumps recorded both when the device is working, but most
importantly when the device has stopped working.
